Dental PPO Network: Humana PPO Traditional 

Southwestern University offers two PPO dental plans. The PPO High Plan has a higher annual benefit maximum, allows you to choose an out of network provider, and offers a one-time lifetime Orthodontics benefit for dependents up to the age of 19.The PPO low plan has a lower premium payment, but you must see an in-network provider.
