Initial Visit

Before your initial visit, paperwork must be completed in order for your session to be held. 

BEFORE YOUR FIRST SESSION (TRIAGE) 

What is a triage session?

The triage session is a 30-minute initial session where you will explore brief mental health history as well as any current challenges you might be facing. From there, your therapist will determine the best course of action for you. Check out our walk-in hours for new appointments below.

Fill out your paperwork ahead of time to expedite your time in your waiting room OR allot 20-30 minutes to complete paperwork when you arrive. Forms required include Client Rights and Responsibilities & Demographics and Personal History.  

Walk-in Hours:

Mondays 1pm - 2:30pm

Tuesdays 10am - 11:30am

Wednesdays 10am - 11:30am

Thursdays 1pm - 2:30pm

INTAKE SESSION

If your therapist determines that individual counseling sessions would be beneficial to you, the next session you will have is an intake session. An intake session is an in-depth exploration of your past mental health and family history as well as your current challenges. The intake provides the counselor with the opportunity to get to know you better so they are able to support you in the best way possible. 

Prior to your session, you will be asked to complete one more document that provides your counselor with more background information. All paperwork is to be completed on our patient portal. The following paperwork is required for the first session:
