Forms for Faculty File Alternative Testing Information for Faculty - Alternative testing can be a reasonable academic accommodation for students with a documented disability per the university’s accommodation process. If there is a student in a course who has been approved for alternative testing accommodations, the faculty member will be asked to submit an Alternative Testing Agreement for the course. This document outlines the steps to submit the agreement to the Center for Academic Success. Note: The agreement will be for the course, not for an individual student.
Alternative Testing Information for Faculty Alternative Testing Agreement Template for Faculty - If there is a student in a course who has been approved for alternative testing accommodations, the faculty member will receive an email within which will be a link to an online Alternative Testing Agreement form in the Accessible Information Management (AIM) System. Through the online form, the faculty member will be able to specify the alternative testing parameters for the course. The form provided here is a hard-copy template of the online form.

Disability-Related Flexibility with Deadlines Agreement Documentation Guidelines - Part of the process for applying for accommodations is the submission of medical documentation that substantiates the presence of a disability under the Americans with Disability Act (ADA). Such documentation must adhere to the guidelines summarized in this document.
Documentation Guidelines Emotional Support Animals (ESAs) - Students with a documented disability may be eligible for an assistance animal as a reasonable accommodation in housing facilities that otherwise impose restrictions or prohibitions on animals. This policy and associated healthcare provider form are intended to specify procedures for requesting an ESA, determining whether the presence of an ESA is reasonable, and specifying the student’s responsibilities for the custody and care of the animal if an ESA is approved.
Please note that students who request an ESA after Sept. 1, 2024, will need to adhere to the updated ESA Policy as revised on Aug. 7, 2024, and provide a completed ESA Provider Request for Information Form as revised on Aug. 7, 2024.
Emotional Support Animals (ESAs) Exam Agreement by Students - All students who use the Center for Academic Success (CAS) for exam completion should have this form on file with the CAS. Student Testing Agreement Exam Scheduling by Students - These are the step-by-step procedures on how to schedule exam completion in the Center for Academic Success. Note: Students must have an account in the Accessible Information Management (AIM) system in order to do this. Scheduling Exam Completion in the Center for Academic Success Housing Accommodations - Students with a documented disability may be eligible for certain reasonable housing-related accommodations in housing facilities. This healthcare provider verification form is intended to gather healthcare information needed by the Center for Academic Success to make recommendations for reasonable accommodations for implementation by Residence Life/Student Housing. Housing Accommodations Request and Authorization to Disclose Information (FERPA) - Through this form, the student can authorize the release of specific information to the listed person(s).
